Vaccine Registration Support

Healthier Kids Foundation is assisting community members 65 years old and older to make vaccination appointments. Their staff is prepared to help eligible Santa Clara County, residents register for their vaccinations and will connect the registrant via text message reminder for their appointment date/time and what to bring. HKF staff will also follow up with any registrant that needs assistance rescheduling an appointment. 


HKF will explain locations, eligibility and answer any questions based on the County FAQ list. The toll-free line 1-833-560-0560 will be covered from 8 am to 5 pm Monday through Friday in English, Spanish, and Vietnamese. ( see attached flyers)


East San Jose Vaccine Location

This week the Public Health Department will be hosting a vaccine clinic at the Public Health Office at 1775 Story Rd. Suite 120 for East San Jose residents 65+. For individuals 65 years or older, Feb 11-12. Please note that people must live in East San Jose, or one of the following zip codes:95111, 95112, 95116, 95121, 95122, 95127, 95133, 95148 (Flyers attached)


Isolation and Quarantine Program

This service is available to individuals who have tested positive for COVID or have been exposed to COVID and need to isolate and quarantine due to exposure For moteling, food, groceries, PPE, and financial supports.

·        Hotline is available 7 days/week, 8 am – 6:00 pm.

·        Hotline is not staffed by a live operator. Customers will hear a voicemail message. You must leave a message with a callback phone number to access services.

Who qualifies for financial assistance (payments of $2000 or $2500) or rental/utility bill assistance (up to $5,000):

·        Residents of unincorporated Santa Clara County and of the following cities: Los Altos, Los Altos Hills, Milpitas, Morgan Hill, San Jose, or Saratoga, whose income stopped or diminished, due to isolating/quarantining, ( Please note this is not a countywide program as some cities are not participating)AND

·        Individuals must qualify based on income of 80% AMI (Area Median Income) or less. (for example, a family of 4 with an annual income of $112,150 or lower) 

·        Financial assistance is available once per household (See the attached information sheet and guidelines)
